- Name, full name
- Date of birth
- Number of your travel passport
- Date and place of issue of passport and data of
expiry
- Preferable terms for being in Ukraine
- Your
current post address, to which we'll send an official invitation
- What Consular
Committee of Embassy of Ukraine
in your country, through which you will easily get up your visas (please, point
out the city in your or other country)
- Your e-mail
and fax number

After receiving
this information we will send you an official invitation and copy to the Embassy of Ukraine
